Forms

Purpose: View, request changes or removal and verify ALL the district created forms for the upcoming school year.

A. Click on Forms

B. View, Change Request, and Verify

  • View each form

  • If changes are needed (including a request to remove), click Change Request and enter information needing to be changed. If needed, an upload if available. Please no PDFs.

  • Verify each form only IF form is ready for the upcoming school year.

C. Review the available content for forms.

D. If a new form is needed, click Form Request and upload new form. Review C to define the first four bullets of the form.

 

Utilize the Optional General Notes, Questions and Additional Information for questions not tied to a specific form. Review this section for questions or comments from the EAI Evaluation Team.

  1. Update the Status to Request Review and

  2. Click Save

  1. Update the Status to Complete and

  2. Click Save.

Once marked Complete, only an EAI Evaluation Team member will be able to reopen.
