How to Reopen a Self-Assessment for an Evaluatee

NOTE: Once a staff member marks the self-assessment as complete and makes available to the evaluator, the staff member will not be able to make changes to it. The SFS staff is not able to reopen a self-assessment. The Evaluator is responsible to reopen the Self-Assessment for the staff member.

  1. Open the Staff Member’s Self-Assessment

  2. Under the Self-Assessment details in the upper left quadrant, click the blue link “Reopen”

  3. In the textbox, you must supply a written explanation of at least 30 characters that provides justification for reopening the observation

  4. The character count will change to the submit button when the requirement is meet. Click “Submit”

A change log will be added into the Self-Assessment for both the staff member and evaluator
